Four Corners Community Behavioral Health (FCCBH) currently provides comprehensive mental health services for adults, youth and children who are eligible for Medicaid, Medicare, and several other insurances who reside in Carbon, Emery, and Grand Counties. FCCBH also provides Crisis Services for all residents of Carbon, Emery, and Grand Counties, regardless of insurance coverage or ability to pay for services. FCCBH is very proactive in finding and developing funding for services for our citizens, so inquire about the availability of services for individuals who have no insurance or other payment source. Four Corners does not deny access to services based on a person's inability to pay. A sliding fee scale is available.
Four Corners also promises not to deny services based on a person's race, color, sex, age, national origin, disability, religion, gender identity, or sexual orientation.
Out-patient services are available for adults. Out-patient services include individual and group therapy, individual and group behavior management, case management, peer support, psycho-social rehabilitation, and medication evaluation and management.
FCCBH provides residential supported living facilities for adults at two facilities, The Friendship Center in Price and The Willows in Moab. FCCBH also coordinates housing placements in privately owned rental units in Price and Moab.
PSR services are basic skill development services provided to adults in an individual or group setting, day-programs, residential programs, or other appropriate settings. These services assist individuals in developing basic living skills.
Out-patient services are available for adults. Out-patient services include individual and group therapy, individual and group behavior management, case management, peer support, psycho-social rehabilitation and medication evaluation and management.
Case management services are available to assist clients in gaining access to needed medical, social, educational and other services. Case management ensures coordination between all agencies and providers involved with a client’s treatment.
Peer support services are available for adults to provide support for recovery and assist with motivation for treatment and compliance with medication.
The Friendship Center and Willows is supported living that is designed to provide housing for up to 10 adults that have been diagnosed with severe and persistent mental illness which greatly impacts their ability to live independently. The goal for residents is to live in the least restrictive environment possible. Residents will be assisted by utilizing side by side work to build life skills that may allow a move to independent housing.
FCCBH offers resources to help members meet their basic needs, such as meals, showers, laundry, mail, computer access, human companionship as well as daily living skills and payee services, assistance in finding housing, employment and education. The clubhouse/day program is staffed by specifically trained personnel and, when possible, by individuals with lived experience who have undergone training. Membership is open to clients who have a mental illness as a primary diagnosis, are more than 18 years old, and referred by the local FCCBH clinic.
Substance Use Disorder
FCCBH provides substance use services for those who suffer from substance use disorder along with mental health disorders.
FCCBH is able to provide services to treat substance use disorders and mental health disorders. We offer evidence based treatment services for all individuals who attend treatment at FCCBH. FCCBH offers Level I (9 or less hours of treatment a week), Level II (9 or more hours a week), Level .05 (prevention Prime for Life). These services include individual psychotherapy, group therapy, case management, and medication management services.
FCCBH is the first rural area in UT to have a fully running OTP clinic where all Medically Assisted Treatment is administered to those who qualify for those services. FCCBH OTP, Operation Recovery is CARF accredited. FCCBH contracts with many residential facilities in UT for those who qualify for residential substance use treatment. FCCBH staff is trained in dealing with those who suffer from Substance Use Disorder and Mental Health Disorders.

Youth and Family Mental Health Services
Out-Patient Services
Out-patient services are available for adolescents and children. Out-patient services include individual and group therapy, individual and group behavior management, case management, peer support, psycho-social rehabilitation, and medication evaluation and management. In-Patient Services
In-patient services are offered to children and adolescents needing psychiatric services when these services cannot be safely provided in an out-patient setting. A multidisciplinary diagnostic and treatment team will assist the individual and family in identifying the need for in-patient services. In-patient services are provided by various hospitals along the Wasatch Front in Utah.
Youth Psychosocial Rehabilitation Services
YPSR are basic skills development services provided to children and adolescents in an individual or group setting, day-programs, residential programs, or other appropriate settings. These services assist individuals in developing basic living skills.
Case Management
Case management services are available for severely emotionally disturbed youth, children, and their families. Case mangers assist them in gaining access to needed medical, social, educational, and other services. Case management ensures coordination between all agencies and providers involved with a client’s treatment.
Medication Evaluation & Management Services
These services are provided to children and adolescents whose treatment includes the need for prescribing and reviewing medication. These services may be provided by a Psychiatrist, Advanced Practice Nurse or a Physician’s Assistant.
